Who Provides Care?
Hospice care is delivered by a team of professionals who will provide well rounded care that includes patient’s medical, physical, emotional and spiritual needs.

The team consists of:

• Attending physician

• Hospice Medical Director

• Family Nurse Practitioner

• Registered Nurse & Licensed Practical Nurse

• Medical Social Worker

• Spiritual Counselor (Chaplain)

• Bereavement coordinators

• Certified Nursing Assistant (Hospice Aide)

• Case Manager

• Intake Coordinator

• Volunteer (optional)

 

Other services available:

• Mobile Doctor

• Mobile Nurse Practitioners

• Home Health Services

• Volunteers/Homemaker (when appropriate)

• Dietitian and therapist (when appropriate)
